Established at the request of the People's Commissariat by Decree of the Presidium of the Supreme Soviet of the USSR on December 22 (USSR Supreme Soviet Gazette, 1942, No. 47).
 
According to its regulation, it was to be granted to all defenders of the city - military men of the Red Army, Navy, NKVD and civilians - always with the confirmation of their actual participation, through the lists of military divisions, hospitals...etc.
 
The defense of Leningrad lasted more than 900 days of siege by the German armed forces, from September 8, 1941 to January 27, 1944, despite the blockade and consequent lack of basic supplies, constant artillery and air attacks and the German advances occupying the suburbs of the city, the Wehrmacht finally withdrew after the offensive of the Soviet troops and the Baltic Sea Fleet in January 1944. On May 1, 1945 the city of Leningrad received the title of Hero City.
 
The Defense of Leningrad Medal was awarded to approximately 1,460,000 people.

There are two variants, the first variant, which began to be awarded during the war, has the ring soldered to the medal, the second variant, which was awarded later to those who had not received the medal at the time, differs from the first in that the ring and medal are of one piece.
