Instituted by Adolf Hitler on January 3, 1944, it was awarded to former members of the Blue Division, Blue Squadrons and volunteers of the Blue Legion.
 
Since the arrival of the Blue Division in Germany in 1941, more than 45,000 Spanish volunteer soldiers fought against the Soviet Union, mainly on the Leningrad front. 
 
In October 1943 it was ordered to disband the Division and return the troops to Spain, between 1500 and 2000 soldiers refused and formed the Blue Legion, which continued fighting until March 1944 when it was ordered to be repatriated. Even so, some remained fighting in various German units, including the Waffen SS until the fall of Berlin.
 
A total of 45,000 medals were awarded.
